As ConnectFamilias continues to grow, we are expanding our current team of dedicated and talented people. ConnectFamilias is funded in part by The Children's Trust, a dedicated source of revenue established by voter referendum to improve the lives of children and families in Miami-Dade County by making strategic investments in their futures. ConnectFamilias, like The Children's Trust, is guided by principles and values that include improving access to early intervention and prevention services to our most vulnerable children, families and neighborhoods, while advocating for and supporting the increased availability of needed services for all children and their families.
Current Openings
Care Coordinator
Works to link youth and families to service providers and to manage care in coordination within a "Team”. Ability to perform these tasks as well as to review comprehensive assessments, services planning and , ongoing intervention, case closure and appropriate documentation and record keeping all within the partnership’s agency’s policy of confidentiality, HIPAA, state and local client privacy rights, and the Partnership’s guidelines. Provides input and feedback to the Continuum of Care Design Team, Participates in Partnership Workgroups and Capacity Building Trainings. Minimum of Bachelor’s degree in Human Services or related field or with a minimum of 2 years experience in service coordination plus some college and experience in community with provider agencies and residents strongly preferred. Bilingual: English/Spanish preferred. Proficient working with ETO and Data Tracker – preferred. Salary negotiable based on experience.
